• IMMUNIZATION REQUIREMENTS


     
    All students are required to be immunized according to state laws and regulations. According to Massachusetts Department of Public Health (DPH) Regulation 105 CMR 220 entitled “Immunization of Students Before Admission to School”, no student shall attend preschool through 8th grade without a certificate of immunization or a copy of school immunization record which documents successful immunization in accordance with Department of Public Health (DPH) recommended schedule.
     
    Preschool students require the following immunizations:
    • Hepatitis B vaccine - 3 doses
    • DTap/DTP/DT/Td/Tdap vaccines - > 4 doses
    • Polio vaccine - > 3 doses
    • Hib vaccine - > 3 doses
    • MMR vaccine- 1 dose
    • Varicella vaccine - 1 dose OR physician-certified history of varicella disease
     
    Kindergarten students require the following immunizations:
    • DTap/DTP/DT/td/Tdap vaccines -  5 doses
    • Polio vaccine - 4 doses
    • MMR vaccine - 2 doses
    • Hepatitis B vaccine - 3 doses
    • Varicella vaccine - 2 doses OR  physician-certified history of varicella disease
     
    Grades 1-6:
    • DTap/DTP/DT/Td/Tdap vaccines -  > 4 doses Dtap/DTP or > 3 doses Td
    • Polio vaccine - > 3 doses
    • Hepatitis B vaccine - 3 doses
    • MMR vaccine - Grades 1-2: 2 doses     Grades 3-6: 2 doses measles, 1 mumps, 1 rubella
    • Varicella vaccine - Grades 1-2: 2 doses    Grades 3-6: 1 dose   OR a physician-certified history of varicella disease.
     
    Grades 7-8 students require the following immunizations:
    • DTap/DTP/TD/Td/Tdap vaccines - 4 doses DTAP/DTP or > 3 doses Td; plus 1 dose Tdap
    • Polio vaccine - > 3 doses
    • Varicella vaccine - 2 doses OR physician–certified history of varicella disease
    • MMR - 2 doses
      Hepatitis B vaccine - 3 doses
       
     
    Immunization Exemptions
     
    When possible, medical or religious exemptions should be documented on Lincoln School district exemption forms. A religious exemption request needs to be submitted by a parent once during the student's school years. A medical exemption request must be submitted by a parent and renewed by the student's physician on an annual basis.  The Commonwealth of Massachusetts does not allow philosophical exemptions.
