Class Placement Information

Family Input Form

Families who wish to provide information to be considered by the placement team can complete this form by Friday, July 17, 2026. The form is not required, and your child’s placement will be carefully considered, regardless. However, please use this form if there is compelling information from your family that you would like us to consider.

Additionally, there are two specific situations for which we request your input:


  1. If you have twins please indicate if you would like them to be placed together or separately. 
  2. If your child won't be attending the Lincoln School next year, please let us know by responding to the last question in this google form.

Requests for a specific teacher will not be accepted. You may indicate if one of your children has already had a specific teacher and you wish for the sibling to have a different experience. This information will be considered but it is not guaranteed that your child will have a different teacher.

Please be aware that for all the reasons described, changes to placement once it is complete are rarely, if ever, made. We cannot make any guarantees due to the number of students and factors that must be considered for the whole school.

You will receive an email in mid-August with the name of your child's teacher!

Health Requirements for Kindergarten

The following requirements must be on file in the school’s health office by August 30, 2026






PHYSICAL EXAM FORM

  • Physical exam signed by a healthcare provider. The first day of Kindergarten is August 29, 2024.  An acceptable physical will be dated August, 2025 or later.


COMPLETE IMMUNIZATION RECORD

  • 5 doses of DTAP
  • 4 doses of Polio
  • 3 doses of Hepatitis B
  • 2 doses of Measles, Mumps, Rubella (MMR)
  • 2 doses of Varicella

Students with a Religious or Medical Immunization Exemption MUST submit a 
respective exemption form (available from Nurse or on LPS Health Services 
website and on registration form)

LEAD TESTING
Documentation of lead testing, since birth, with results and the date 
recorded.

MEDICATION
If your child will require administration of any medication during the school day, please contact the School Nurse prior to the start of school.

Records Request

Dear Caregivers,

Thank you for completing the Lincoln School Family Information and Orientation Signup form! 

Please provide your child’s most recent preschool progress report and/or a Kindergarten Transition Report (details below). These insights allow our faculty to understand your child’s unique learning profile and ensure a seamless transition into our classrooms.

Report cards can be emailed to Catalina Gonzalez, Administrative Assistant, at cgonzalez@lincnet.org; or mailed to Lincoln School, ATTN: Kindergarten, 6 Ballfield Road, Lincoln, MA 01773.

The Kindergarten Transition Report has two parts. Part 1: Student Information is completed by you, the caregiver. Part 2: Transition Report is completed by your child's current teacher and mailed back to Lincoln School, ATTN: Kindergarten, 6 Ballfield Road, Lincoln, MA 01773.

Registration Information

Employee Children
Students who are children of Town of Lincoln employees must register but do not need to provide residency documents.

Lincoln Preschool
Students attending Lincoln Preschool do not need to register. They are already enrolled with LPS.

METCO Enrollment
METCO participants need to register but do not need to upload residency documents or a birth certificate. These are already in Avela. Health documents must be uploaded or sent to the school.

Private Preschools
Students enrolling from a private preschool need to register and provide all required documents (health, medical, residency, birth certificate.

Summer Reading Programs

Summer Reading 2026 at the Lincoln Public Library!

★PROGRAMS: There’s something for everyone at the library this summer! Thanks to the amazing support of the Friends of the Lincoln Library, we’ve got a jam-packed schedule full of storytimes, hands-on workshops, music, cooking, and more! 

★SUMMER READING CHALLENGE: The library will also be offering our Summer Reading Challenge. Starting June 17, pick up a Summer Reading challenge form at the library or download one online
  • Pre-readers - Grade 1 earn a sticker for each challenge completed and add it to a complimentary sticker book. If ALL CHALLENGES ARE COMPLETED, participants get to pick a prize from our prize basket. 
  • Grades 2–6 earn a button for each challenge completed to be added to a complimentary lanyard. If ALL CHALLENGES ARE COMPLETED, participants earn a raffle ticket towards one of FOUR big prizes to be drawn at the end of the summer.
  • Grades 7–12 earn a prize for every six completed challenges. If ALL CHALLENGES ARE COMPLETED, participants will earn a raffle ticket towards one of two big prizes – either a brand new Kindle Paperwhite OR a $50 gift card to Concord Bookshop.

★SUMMER READING BOOK LISTS: We take great pride in creating suggested Summer Reading Book Lists. We review books published in the last year as well as perennial favorites with something for everyone. Check out our Book Lists here. 

★EBOOKS AND AUDIOBOOKS: Books can be accessed digitally with a library card. Students who do not yet have a Minuteman Library Network card can get an ecard using this link:
